Don't refer to NCTH unless you are prepared to do EVERYTHING yourself. It's exhausting and they do the bare minimum to keep you in their service.

After being on the waiting list for 5 years I got my first appointment Jan 2024. I had a second appointment June 2025 where I was asked to arrange bloods with my GP.
Two weeks later the bloods were sent off to NCTH and as I was told by the clinical that it would be around a week before I got my HRT prescription I called a month later after not hearing anything.

I got told that my clinician was on holiday and that they had also lost my report. So I went and got new bloods. Four times.
March 2025 my clinician finally was able to look over bloods I had done a few days before and arranged an appointment.

At the end of this appointment they said they were going to send a letter to my GP requesting my HRT.

This wasnt sent. I now have to call NCTH and request a copy of the letter to send to my GP myself which has made this process twice as long as it needed to be.

Horrendous service. Called to ask about a surgery referral letter and when I asked if I would be able to get it within 6 months I was met with what could only be interpreted as a scoff and a laugh. The fact that arrogant people with no empathy are put in positions to work with trans people is disgusting and shouldn't be allowed! Waiting years for treatment isn't an option for a lot of people and at the very least those answering phone calls should understand the importance of these things rather than lecturing about how there are wait lists and they need time to do everything properly. It's all ridiculous time wasting to punish people for being different.

When I first went under the care of the Nottingham Gender Clinic in 2014 their care was absolutely first rate. However, I have to say that now in 2024, it is nothing short of atrocious. For the past 15 months or so both myself and my G.P. have been continually asking for an appointment with one of the endocrinology doctors to advise me on the correct dose of Estrogen stressing to them that my symptoms are just getting worse and worse. As it is I have been told that due to waiting lists, I should treat myself based upon how I am feeling and if that is not sufficient then perhaps I should consider seeking out a private appointment. I feel the next step for me is to raise this issue with PALS.

Advice to those entering this system from someone it deemed invalid and unworthy of support:

This system was created by neurotypicals and you dear reader probably aren't. Like all neurotypical creations the system is arrogant, inflexible, denies agency to it's users(by which I mean the doctors) and concentrates power along with responsibility into a central canon of rules that are sacrosanct.

The doctor needs to be able to tick some number of boxes on a check list in that canon, if they can't do that, you do not exist. This is something that cannot be understated, the book of rules is a holy book and you must conform if you want support.

So you are probably going to have to lie, portray yourself as the stereotype that the inflexible system wants to see and further reinforce the systems delusion that it knows best.

I have been under the care of the Nottingham Clinic for approx. 7+ years and while it took me several years to get onto the waiting list, once I'd gotten my first appointment I couldn't be happier with the service I've received.

Everyone is very nice and professional and the longer I have known the clinician I have worked with, the more casual (but professional) it has become. Which works great for me as I can be a bit cagey when someone is overly clinical.

There have been times when appointments have been cancelled, but I've always been told with a few days notice minimum and when it's been less, I have received a lot in the way of understanding.

On the flipside, when I have forgotten appointments myself (or had a technical issues once switched to online) they have been exceptionally understanding and have still made the effort to reach out and get all the key information from me, so nothing important is lost.

I had no issues with any prescriptions and have made contact with my local pharmacy/GP so my medication has always been regular. (Apart from major supply chain shortages, but no one is a magician)

For anyone who might be anxious by the reviews, while this is my personal experience, I have found that as long as I remain open and honest about any challenges or issues on my end, once they've got you on their systems, they will do what they can to support you. My last in person visit was a few years ago so this may have changed, but for the small team and establishment they work from, I am incredibly impressed by how they have treated me.

Many people go into this feeling defeated and scared, but without the clinic's support, I would not be where I am today and I am incredibly pleased that I made the decision to refer myself to them.

Update 2 : 05/25
Had 2nd appointment and honestly can't fault Dr. Robinson, was really helpful in answering any questions I had in regards to moving from injections that i source myself to actually being prescribed by them. Have been told that a request for bloods along with initial request for a prescription will be sent to my GP so will just wait and see how long that takes. Overall though once you get past the wait times they are really helpful and kind, at least in my experience. They are obviously aware of the wait times as the doctor had stated that my next follow up appointment will most likely be over 6months even if they request it for 6months but that is simply down to a underfunded system. Rating for the doctor i saw would be a 10/10 though, they listened and explained things better than any other doctor I've ever seen through my GP. Will update again once the prescription comes through to GP and any other changes with the service. Definitely gets better once your getting seen so just bare with the wait times as frustrating as it is, it isn't the people whom work there who are at fault so keep that in mind.

Update 1 : 11/23
So over the last few months of phoning them back and forth, which has improved greatly since I got through to a person every time, I found out my referral had been lost. So after alot of going between them and my doctors to get it sent back through they have agreed to honour the original referral date (I did mention if they didn't I'd be taking it up with the local ombudsman). Received and initial appointment for 12/23 so that'll be about 3yrs since my original referral. The staff I've spoke to so far have been lovely and helpful hence going from 1 to 2 stars. Will update again after first appointment.

I'll be updating this review once I actually get seen but currently very unhappy that they can't even be bothered to answer the phone! In comparison I had rang the London GIC by mistake and they had answered after a 2min wait. Totally unacceptable that they have nobody available to even take patient phone calls.

It was appalling, degrading, evasive with so many personal questions, disappointing, depressing, disgusting, triggered many flashbacks, my PSTD, panic attacks, repetitive as it was in the previous Online video appointment, not punctual, kept waiting for more than 20 minutes late from the given appointment time, they are lack empathy, sympathy, and professionalism.

I'll be honest, I was very worried by all the negative reviews.
My experience couldn't be any more different, however.

I waited a bit over 2 years for my first appointment, which is a long time, but still one of the best wait times in the country.

My second appointment was supposed to be nearly a year after that but was moved forwards by 6 months.

As I was already on HRT privately with GenderGP, the doctor agreed to send out a letter requesting my GP to handle my medication on my current private dose right away.

That puts the total time from referral to medication letter at 2 years and 9 months for me.

The doctors have been absolutely lovely, professional, understanding and respectful. At no point was I asked inappropriate questions (beyond maybe my sexual orientation) and I never felt like I had to justify myself.

I was however informed that 2 speech therapists recently stopped working for the NCTH and therefore waiting times for speech therapy are now at 11 months - Very unfortunate, but of course not the fault of the NCTH. I was also informed that this waiting list may reduce drastically once they find more speech therapists.

They can sometimes be a bit difficult to reach via phone, however it seems much better than it was over Covid, judging from reviews from that time.

I will most likely update this review based on how long the medication letter takes to arrive - Already been waiting 2 weeks now.

All in all, the NCTH is probably one of the best functioning parts of a broken system.

Go up
